Fifteen minutes of paperwork, done before you arrive instead of in our waiting room.
Every new patient completes a short set of forms covering your contact details, dental and medical history, current medications, insurance information and consent to treat. None of it is long, and all of it matters — your medical history in particular changes how we treat you safely.
Ask the front desk to send your new patient pack when you make the appointment. Fill it in at home, at your own pace, with your insurance card and medication list next to you.
Turn up a quarter of an hour before your appointment and complete everything at the office. Bring your insurance card, photo ID and medication details.
If you would rather go through it over the phone, or you need anything read out or explained, call (408) 241-4111 and we will take it down with you.
Why we ask about medical history: blood thinners, bisphosphonates, diabetes, heart conditions and pregnancy all genuinely change how — and sometimes when — we treat. Telling us upfront prevents a cancelled appointment on the day.
Your records and health information stay confidential and are used only to plan and provide your care, bill your insurance where you have asked us to, and meet our legal obligations. We do not sell or share patient information.
Ask your previous office to forward your records and recent X-rays. Most send them within a few days, and it can save you having images retaken at your first visit. If you are unsure how to request them, tell us and we will handle it.
Book first, then complete your forms — we will send them straight after you call.
